Important — please read
This is not health insurance. Under the Arkansas Direct Primary Care Act (Ark. Code § 23-99-103), Rooted Health memberships are direct primary care agreements. We do not bill Medicare, Medicaid, or commercial insurance. Maintain separate coverage for services outside this practice's scope. Hormone and GLP-1 management require the matching specialty plan; included services are provider-directed and subject to clinical appropriateness. All prescribing — including controlled substances and weight-management medications — follows Arkansas State Medical Board and DEA rules and requires an in-person or telehealth evaluation as required by law.
Not included in any membership: pap smears / gynecological annuals, specialist or referral costs, emergency or urgent care, hospital or surgical care, imaging, labs beyond the included panel (billed at member rate), insurance billing of any kind, and medications outside your tier (member discounts apply).
Cancellation: all memberships are one year; 30-day written notice required (in person or certified mail); early cancellation incurs a $150 fee; fees are non-refundable for the current billing period.
